Video Transcript: The skill set we have at eMerit is to help de-escalate conflict. By de-escalation, I mean to help you respond directly to the patient, if you know who the patient is, or to respond to the public at large. When a doctor calls us and says they received a negative review, typically if they are a client, we will find it before they see it. So our task is to have a robot that goes out and checks the different sites and we will be aware of a negative review. But, regardless, the philosophy is the same. Typically, it's just listening to the story and making it right. It doesn't require rocket science to do that. But you've got to ask the right questions. So we suggest reach out to the patient and see whether you can make it right. That's going to solve many different problems. In fact, it'll solve most problems, most of the time. In the plastic surgery world, the patient may not be happy with his or her subjective outcome. Sometimes it requires some hand-holding to say be patient, I know that this is frustrating, but often over time, you'll like what you see. If you will just be patient. But sometimes, the patient will never be happy and you may want to offer a revision procedure, either at a reduced or at discounted or at no price whatsoever. I mean, these are tools that can be used to make it right, to try and exceed the patient's expectations. Most patients don't expect this, most patients don't demand it. But the average doctor's defined online by four to five people. The problem with just having a handful of reviews is that from a statistical standpoint, you may not like what you see. So the dynamic calculator was a tool that we created to help demonstrate that you need lots of reviews. You don't want to be defined by a handful of reviews. You don't want to be defined by just five reviews for example. You want to be defined by hundreds of reviews because hundreds of reviews would be a representative picture of who you are. You leave it to chance you're going to find out that it may work out OK but odds are among a hundred different doctors, a subset of those doctors are not going to like what they see even when they're doing a great job. The law of large numbers suggest that you will be defined appropriately if you have a large number of reviews. If you see 1 to 3,000 patients a year, you don't want to be defined by three or four patients. Now you may get lucky. But why leave it to luck? In an attempt to prove believers wrong, he set out on a research project that surprised even him. Video Transcript Are reviews valuable and can people use reviews on the Internet to find a great doctor? I was a skeptic, I was a public skeptic about the value of reviews. I went out of my way to demonstrate just how valueless reviews on the Internet were. So how did I do that? I got a list of the best and worst doctors for things that mattered the most which would be patient safety or "don't kill me" and clinical outcomes or "make me better." And so I had a list of the best and the worst surgeons in those two categories because I would think that that's what I would be looking for in terms of a practitioner. I mean it would be nice to have great bedside manner, good communication skills, great parking, ease of scheduling, all that is nice to have but the must have is patient safety and clinical outcomes. So I got this giant list of the best and the worst surgeons, but I didn't know which was which. I was blinded as to the identities. And then I had some interns go to the Internet and scour page one of a Google search for reviews; what was the online reputation of each of these doctors like. We went to Health Grades, we went to Vitals, we went to Google and so on and so forth. And then when we unblinded the information it turned out that the best surgeons had the best online reputations. The worst surgeons had the worst online reputations. My assumption was completely wrong. That reviews and online reputation do matter and they are a very useful signal in terms of how to find the better doctor. That was the epiphany in terms of establishing e-Merit. If reviews online are helpful to give patients actionable information, why should a doctor be defined by two angry patients with a megaphone? Shouldn't they be defined by hundreds of patients since the average doctor sees several thousand patients a year? The answer of course, is yes. Greetings, my name is doctor Eric Joseph. I'm a facial plastic surgeon and adult otolaryngologist practicing in West Orange, New Jersey. We've been in solo private practice for fourteen years. When I started in private practice in 1999 the majority of my referrals were sent to me from either primary care doctors or from other specialists. Today things are quite different in several regards. One, the majority of our practice is focused around facial plastic surgery as opposed to being primarily focused on otolaryngology and we're also a third party free practice which has been a slow transition over the last three and a half years. Ninety percent or more of our patients end up finding us on the Internet. At least right now the Internet is a blessing because it ends up reaching a much larger catchment area. We get patients from all over the United States of America and even internationally. The challenge of the Internet, the challenge that a physician faces when dealing the Internet is controlling it. Anybody who's watching this video that's a physician, if you haven't put your name into a search engine and Googled it you should do it because you'll end up seeing right away on the first page and the second page that your name in physician rating sites is going to come up. If you have a disgruntled patient or an unhappy patient they are very likely to broadcast their negative experience over multiple physician rating sites and sometimes reputable physicians can have their reputations tarnished and dominated by a small minority of patients who are unhappy. My experience has also been that happy patients, unless it's made easy for them, your happy patients are unlikely or less likely to spontaneously go to the Internet and opine about their wonderful experience that they had with you. So what e-Merit has been able to help us with is enabling us to capture this happiness at the point of service while they're in the office at checkout. Those reviews and comments, having them populated on the multiple physician rating sites that exist today has really been a blessing because it's been able to put a fair perspective and a fair analysis of what my actual patients actually experience when they leave my practice as opposed to the whole narrative being dominated by the disgruntled few or the fraudulent and fictitious posts. Once you implement it into your practice it just sort of takes over on its own and the Internet just starts populating with what your actual patients are saying. They're here because they read the reviews that our patients have expressed and they're here because they see the before and after pictures that I've posted on the Internet and it's really the combination of Internet reviews, before and after photos and just the knowledge that you're able to disseminate these days on the Internet. You get less stressed out by the fraudulent posts, which I've seen multiple times and you get less stressed out by the occasional discontented apple that you just can't make happy no matter what you do because there's a continual stream of your own actual patients that keep populating the sites and that I think is priceless. My name is Joseph David Stern and I'm a neurosurgeon in practice in Greensboro, North Carolina and my practices name is Carolina Neurosurgery and Spine Associates. I really enjoy my relationship with patients and particularly I really enjoy the field of neurosurgery. There are many different features to it, there are many different venues. I see patients in the office, I do surgery, I work in the hospital, I deal with hospital administration. I really like the pace and I like the variety of the practice. I think I've probably been using e-Merit for about four years. Probably, maybe even a little longer. Initially it sort of sat there, I didn't really use it that much and now I really do make it a part of my practice and I find it very gratifying. So I will have a patient who has had a positive experience or actually one lady the other day said, "I want to say thank you. How can I say thank you?" And I said, "Well here's a way you can say thank you. If you do an evaluation of me this is helpful for my practice." And so usually at the conclusion of the episode of care or after someone has come back and has done well with surgery I'll ask them if they don't mind doing an evaluation. I went to Lowes and bought a refrigerator and a lady there who sold it to me said that I would like for you to give me a review. And in the end I wanted to give a review but I left the store and I never really did the review. So, I think the fact that I have a mechanism to give a patient an opportunity to give me a review right then and there means that I'm getting a lot more reviews than I probably would. I think that I was initially skeptical of whether it would be of value to me and I really have found it to be very valuable. It kind of hit home the other day when I had a patient who came from three hours away for surgery. She had been recommended a specific surgery and I recommended a slightly different surgery but she had come to me because she had found me online and thought I had a good reputation. So, she sought me out it wasn't a physician that referred her. She came, she actually had the surgery, she did very well, she wrote a review that said I wanted to thank you and that I had lived up to my online reputation. So that was kind of a, for me, validating the whole e-Merit platform. That not only had she found me through the Internet but then she wrote and said that I had lived up to my Internet reputation. So, right now I think that patients are still finding me in the same way by word of mouth and also by doctor referral. I think the big difference is that the Internet is a tool that patients use to validate me and to confirm that, yes, I really do want to see this person. I think also the word of mouth referral through the Internet has expanded so it's not directly people that these people know but that they go online when they are told they a neurosurgeon to find is this someone they should see or sometimes I'm actually seeing patients that have been referred to other doctors and they say, "I really didn't want to go to that person" or "I went to that person and I didn't really like them." So, they show up on all kinds of websites and I'm finding them on more doctor review sites I didn't even know existed. You Compare is something I didn't know about. A lot of the times they'll show up on Vitals, they show up on Health Grades, they'll show up on Google but many, many different sites. But I've been named at Top Doctor, Top Regional Doctor and I keep getting these awards, Compassionate Doctor and a lot of positive feedback from these review sites. I would be delighted to speak to anyone if they had any questions. I think that it is, it's really not doing anything different than your patients would do ordinarily but it's giving them an opportunity to express their feelings. And the thing I like about e-Merit is that if they're saying negative things I get some feedback on that. It's valid, reasonable, feedback and with feedback you can make some constructive change so sometimes I find maybe not all the reviews are fantastic but they give me an opportunity to kind of reflect on what I'm doing.
